Application Process

Welcome to Judevine Center’s Admissions Department! Here are the steps you will need to carry out in order to access Judevine Center Services:

Complete a Judevine Center Application. Either download the Judevine Center Application by clicking here, or contact the Admissions Department and have one mailed to you.  After completing the Judevine Center Application, please refer to the bottom of the third page of the Application and provide all available information as is appropriate for the applicant. It is especially important to provide a copy of the applicant’s Social Security Card* as well as a copy of a document that verifies the applicant’s diagnosis*. This can be a document from the applicant’s diagnosing specialist or a copy of the most recent IEP from school. (An applicant does not need to have a diagnosis of any kind to access Judevine Center services or to benefit from them.).
*In order to be considered eligible to receive selected grant funding opportunities available for certain consultation services provided through Judevine Center, this information is mandatory, but is not a guarantee for access to grant funding.

• Submit the completed Judevine Center Application. This can be done by:

• Upon receipt of the application (and attachments), the Admissions Specialist will contact you to verify its receipt and to discuss the next step(s) toward accessing Judevine Center services.
